Just a few I don't think have been covered yet. In MASH Trapper (Wayne Rogers) was replaced by BJ (Mike Farrel). The character Henry Blake was replaced by Henry Morgan's Sherman Potter. The guy who played Blake was reportedly unhappy with the way they wrote him out (death) since it closed down the option of returning. Oh, and the nasty whiny Frank Burns was also replaced by the snob Major Charles Winchester.

In Drop The Dead Donkey Haydn Gwynne was replaced by Ingrid Lacey.

And, I'm not sure it counts, but Chris Langham's character in The Thick of It was replaced in the Christmas specials by an amalgam of other characters. (Anybody know why they replaced Langham? I'm still not sure?)
