Everything is premise and characters. The rest is just scribbling.

I tend to use four acts.

Act 1 sets up the scene

Acts 2 and 3 create the issues, obstacles or problems for the characters

Act 4 is the resolution

I actually think there are no hard and fast rules, and as a writer it never hurts to mix up the way you conceive and write things to keep you from getting stale.
