The person driving is really short and can't see through the window
Or
They have a paper bag over their head and the caption reads
"Ugliness affecting your day to day life? Get plastic surgery and you'll be able to take your bag off"

It's a child

Good suggestions. A spooky child WITH THE FACE OF JEREMY CLARKSON might be the way forward.

Better still: shots of a man shouting instructions like "Left hand down a bit" "Whoa whoa whoa" at a van. Cut to his viewpoint and we see he's actually shouting instructions at doggers inside :-)
