I'm not a smoker, so I'm not really sure how it's done.
Usually you put it in your mouth and smoke it. There are some ladies in Amsterdam that do tricks with cigarettes and other orifices though*, so maybe Lee did use his ears before in the old days when he was a naughty monkey.
*I don't know if this is true not having seen it with my very own eyes.
