Yes, you did read it right.

They say you're allowed three/four sets max on a studio audience comedy but does that include rooms going off from the main room? For example, in Frasier if they use the coffee shop, the radio studio, the apartment and the kitchen. Is that counted as four or three because the kitchen is part of the apartment? Is the hall in the radio studio counted as one or just part of the radio studio?

The studio and the hall will be one set. They're joined, as you can see when they walk from one to the other.

in studio sitcoms how do they do stuff that happens outside or not in the regular set?

They'll build a set if it's done indoors.

It's outdoors it's done on location and shown to the audience on a big screen at the relevant time in the studio.
