Yes I remember that. In the original pilot episode which is available on the Series A DVD there were no monitors at all, so they had hand-writtern cards. The on-screen captions appeared as of the second episode, and the cards were used up until the third series if I remember rightly.
Also the siren went off more often in the earlier series.

Yes. I'm guessing that is because the panellists are now so aware of the forfeits that they don't want to ring them.
They should be made to. That was half of the fun.
Except it was kind of irritating when Robert Webb did it on purpose.
That klaxon sounds is one of the most annoying things on TV!
