It varies from country to country, ethnicity and tribe. In some places it is indeed the barbaric removal of the entire labia minora and the clitoris. Or the dangerous practice of infibulation. In other cultures it's only a ceremonial prick with a needle (no mutilation or surgery) to signify that a girl can enter womanhood.

Very few boys will reach adulthood and decide they want to get the end of their knob hacked off! Some may do it for religious reasons, especially if they are marrying a serious Muslim or Jewish girl and have to convert to her religion. Baby boys won't remember or complain or suffer long-term damage if circumcised when they're babies. Jews have the right idea - if it must be done, do it when the baby boy is 8 days old. I feel sorry for the Muslim boys who generally have to wait until they're 13 to get circumcised as a necessary rite of passage/religion - as they will remember the pain and discomfort.
