The basics of how cancers arise are indeed quite straightforward. The tricky bit to get your head around is exactly how and why they spread and the exact reasons they can kill you. Then it can become rather complex and confusing. For example, the question "Why does cancer kill people?" is quite a difficult one to explain. (Which is why you won't see the health sections of the newspapers trying to take that one on.)
Does cancer affect the blood stream or is it normally a case of putting pressure on a vital organ? I know it depends where it is, obviously but do all non-benign cancers spread?

The ultimate way that cancer actually kills is very complex, but it basically comes down to the biochemical and metabolic effects of the numbers of cancer cells weakening the rest of the body and causing the organs to fail. Of course, people can be killed by the anatomical effects of certain tumours, though this is less common. All malignant cancers tend to spread (as indeed do a few benign ones), but at different rates depending on where they are and the type of cancer cells. Cancers tend to spread via the lymphatic system prior to via the bloodstream. Again, the original site of the cancer and the cell type determines the speed and extent of spread. The most important point really is that there are many different types of "cancer" cell, any of which can arise from the same primary location. This is why looking at them under a microscope is so important in terms of choosing treatment and predicting survival.
