And if these people had access to a welfare system for social and medical care they wouldn't need to bear as many children.

It's very much a cultural thing. We're talking about a region where thousands of young girls are raped every year because the men superstitiously believe that having sex with a virgin will cure AIDS. There is a lot of work to be done to change the views on sex, procreation and health care in that part of the world.
